To enroll in the wellness plan, contact the Wellness Team at (904) 474-3158 or Wellness@HRDelivered.com. A
team member will share your net pay impact and review any plan information. After initial enrollment, there are quarterly open enrollments for this plan.
The deadlines to enroll are 2/15, 5/15, 8/15, and 11/15. You will see deductions begin the following month (3/1, 6/1, 9/1, 12/1), and will be fully effective on the plan after a month of deductions (1/1, 4/1, 7/1, 10/1).
The wellness plan is a pay-ahead program with a plan effective date of the first of the month following the month that wellness plan deductions appear on your paycheck. Once your plan is effective, you will receive login instructions to access your account for the first time.

The plan has a Section 125 tax benefit, and there will often be $0 out-of-pocket costs due to tax savings from the tax-advantage benefit., which often cover the plan’s cost. As long as your tax savings are over $190/month, you will have no cost associated with this plan- in fact, you may receive extra tax savings in your take-home pay!
PLAN Loss Fund: This is a pre-tax deduction that lowers enrollees’ tax liability; usually offsets the plan’s cost, often leaving employees with a positive net impact on their paycheck.
PLAN Premium: This is a post-tax deduction that covers the cost of health benefits like in-person care and telemedicine.
PLAN Benefit: This is a credit that offsets other deductions; it is non-taxable as it’s considered an insurance benefit.
The average net pay increase is $125 per month but the tax savings amount varies based on gross pay, W-4 elections, and other deductions. You will need to contact the Wellness Team to review this information prior to enrolling.

All information is available in both English and Spanish. You can access your Summary of Benefits through the Resource Center in your app account, after your effective date.
This plan can act as a minimal essential coverage (MEC) plan, but keep in mind that there is limited coverage. Primary Care, Urgent Care, and Pharmacy have covered services, while specialty appointments and hospitalization are not covered.
Yes, the wellness plan is designed to complement primary health coverage.
No, PCP specification is not needed. The provider only needs to be in the First Health network.
You’ll need to complete an activity within 60 days or risk being taken off the wellness plan. We also send out emails to help you stay current each month. Keep in mind, this does not opt you out of the program. Reach out to the Wellness Team for enrollment support.
